This plan is designed for patients who want support, coordination, and monitoring — without unnecessary visits or delays.
What the Care Continuity Plan Includes
Same-Day Nurse Texting (8am–8pm)
Text directly with our nursing team using a dedicated nurse phone number. Messages are reviewed and
responded to the same day during extended business hours.
Texting is asynchronous and designed for guidance, monitoring, and next-step planning — not emergencies.
Common reasons patients text:
New or ongoing symptoms
Lab results and trends
Medication timing or side effects
Weight-loss check-ins
Follow-up after ER, urgent care, or hospital visits
Care coordination questions
Paperwork or documentation support
Daily or weekly symptom updates
If something cannot be safely handled by text, your nurse will guide you to a nurse visit or provider visit when appropriate.
Nurse-Led Acute Care (Under Standing Orders)
When appropriate, nurses may evaluate and initiate care using clinic-approved standing orders — without requiring a provider visit.
This may include care for:
Urinary tract infections (UTIs)
Sore throat or strep testing
Flu-like illness or COVID testing
Pregnancy testing
Testing, treatment, education, and follow-up are provided according to protocol. Provider review is required when symptoms fall outside standing orders or safety concerns arise.
Discounted Nurse Visits
$30 per visit (in-person or telehealth)
When hands-on care or real-time assessment is needed, members receive deeply discounted nurse visits. These visits may include:
Focused assessments and vitals
Point-of-care testing
Lab review and education
Weight-loss follow-ups
Injections or IVs (when ordered)
Care planning and documentation
Lab Access & Member Discounts
Members receive 10% off lab testing, including panels and individual labs.
Many common labs are available without a provider order
Nurses help guide lab selection and review results
A standard $20 draw fee applies per blood draw
Lab results are reviewed with guidance on next steps and provider follow-up when indicated.
Ongoing Symptom Tracking & Health Monitoring
Some health concerns require time and pattern recognition, not one-time visits.
After a provider establishes an initial plan of care, nurses can:
Track symptoms over time
Create custom daily or weekly questionnaires
Monitor response to treatment or lifestyle changes
Coordinate follow-up labs
Prepare summaries for provider review
This supports more informed, data-driven care decisions.
Weight-Loss Support
Weight-loss support is a core part of the Care Continuity Plan.
Nurses provide:
Ongoing check-ins
Education and accountability
Lab monitoring guidance
Coordination with providers when medication changes are needed
Medication decisions remain provider-directed, while ongoing support is nurse-led.
Patient Advocacy & Paperwork Support
Healthcare can stall when coordinating with employers, schools, pharmacies, specialists, or caregivers. Your nurse can help:
Clarify questions for other providers
Coordinate records and timelines
Assist with documentation and follow-up
Support communication when care feels stuck
ESA & FMLA Support
Nurses can assist with:
Gathering history and documentation
Administering questionnaires
Preparing paperwork
Coordinating provider review and signatures when criteria are met
Most ESA and FMLA paperwork can be completed without a separate provider visit, when appropriate.
