You'll receive a full evaluation, time to discuss your health concerns and goals, and a customized care plan developed collaboratively. We’ll also review any current medications or supplements and outline follow-up steps or additional services you may benefit from.

Ongoing visits for established patients who need follow-up care, medication management, chronic condition support, or lifestyle coaching. These visits allow time to review labs, adjust treatment plans, and stay on track with your health goals.

• Anxiety, depression, and ADHD

• Hormone health

• Digestive issues

• Autoimmune and inflammatory conditions

• Preventive care and wellness support

Visits are available in-person or via telehealth for patients located in Utah.

We offer:

• LDS and other missionary clearance exams

• Youth sports physicals for school or club sports

• BSA-compliant scout physicals (Parts A, B, and C)

Evaluation – $120
We provide professional, confidential evaluations for patients who qualify under Utah’s medical cannabis program. Our licensed providers guide you through the state’s Electronic Verification System (EVS) and ensure your application process is smooth and compliant.

Qualifying conditions include:
