Available Appointment Types

• New Patient Visits

  • To reserve your new patient appointment, please call the office at (435) 799-2581.

  • New patient visits are $200, and this amount is paid at the time of booking. This payment secures your appointment and pre-pays for your visit.

  • The reservation payment is fully refundable when cancellations are made 24+ hours in advance.

  • Appointments canceled with less than 24 hours’ notice, or no-shows, are non-refundable. The full $200 is forfeited because that time was reserved exclusively for you.

  • This policy helps ensure availability and timely care for all patients.

• Continued Care Visits - $85 to $150 depending on complexity

• Medical Cannabis Evaluations - $100 Renewals - $150 New Evaluation

• Missionary Physicals (including blood work and PPD test_ - $80

• Sports and Scout Physicals - $40

• Lab work/Blood draws - varies

• Nurse Visits - $60

• IV’s - varies

Need to cancel or reschedule? No problem—just give us at least 24 hours' notice. Appointments cancelled with less than 24-hours-notice will be charged the full appointment fee.
